Specialized kratom-specific tests can detect metabolites for 5–7 days after a single use and potentially 1–2 weeks for heavy regular users.

Does kratom show up on standard drug tests?

No. Standard 5-panel and 10-panel SAMHSA urine drug tests do not screen for kratom alkaloids. These tests look for THC, cocaine, opiates, PCP, and amphetamines (5-panel) plus additional substances like benzodiazepines and barbiturates (10-panel). Kratom metabolites are not included. However, specialized kratom drug tests do exist and are used by some employers, probation programs, and courts in states where kratom is regulated. These tests detect mitragynine and its metabolites. If your testing situation is ambiguous, ask specifically whether a kratom-specific panel is being used.

Can kratom cause a false positive for opioids?

Very rarely, and only with low-quality immunoassay tests. Kratom's alkaloids are structurally distinct from classical opioids and do not typically cross-react with standard opiate immunoassay panels. Several case reports in medical literature have documented false-positive opiate screens with kratom, but these are exceptions. Mass spectrometry (GC-MS or LC-MS/MS), the confirmatory test used when an immunoassay is positive, will definitively distinguish kratom from opioids. If you receive an unexpected opiate-positive screen and use kratom, request GC-MS confirmation. Most labs perform this automatically, and kratom will not confirm as an opiate.

How long does kratom stay detectable if tested specifically?

Detection windows for kratom-specific tests: urine (most common) — single use: 5–7 days; regular moderate use (daily): 7–14 days; heavy chronic use: up to 2+ weeks. Blood tests: shorter window, typically 1–3 days after last use. Hair follicle tests: potentially up to 90 days for heavy users, though kratom hair testing is rare and not yet standardized. Saliva tests: very short window of 1–2 days. Body fat percentage affects elimination: mitragynine is lipophilic (fat-soluble) and can be stored in fat tissue, extending elimination in people with higher body fat percentage.

What factors affect how quickly kratom clears?

Key variables affecting kratom clearance: dose (higher doses = longer elimination), frequency of use (daily use allows accumulation beyond single-dose kinetics), body fat percentage (fat storage extends elimination), age (liver enzyme function decreases with age, slowing metabolism), hydration (adequate water intake supports renal excretion of metabolites), and liver health (kratom is metabolized by CYP enzymes in the liver; impaired liver function significantly extends elimination). Co-ingestion of other CYP3A4 inhibitors (certain medications, grapefruit juice) can also delay mitragynine metabolism.
